Planning and Construction Committee Minutes
October 20, 2004
A Meeting of the Planning and Construction Committee was held on Wednesday,
October 20, 2004 at 7:00 P.M. at the Beverly Public Library, 32 Essex Street,
Beverly, MA.
Members Present: Chairman Ray Bouchard, Rod Maxner, Cheryl Sydorko, Dan
Skokski, and Tim Brennan.
Others Present: Arthur Diagnault and Cheryl Lamont.
The Meeting was called to order at 7:00 P.M. by the Chairman, Mr. Ray
Bouchard.
Mr. Skokski made a motion to approve the minutes of September 15, 2004 as
presented. Mr. Maxner seconded. All in favor. Motion passes.
Chairman Bouchard states that a price for the skylight for the Beverly Farms
Library has been finally obtained and will be completed before winter. The
plaques were installed today.
Chairman Bouchard states that the Fuel Tank Remediation Project is now over
and a local group has made plans for a walkway between West Street.
Chairman Bouchard states that the tank removal has been completed at the
Beverly High School and grass is growing.
Chairman Bouchard states that the Boiler Project has finally been completed.

Chairman Bouchard states that the amendment for the investigation and
feasibility study of Beverly Golf and Tennis Club House is in the Mayor’s Office for
his signature. They will be working on getting a new contract for the
feasibility/schematic design and eventual design of the maintenance building
replacement at the end of Goodyear Street and will most likely be a prefabricated
building.
Chairman Bouchard states that he just received a draft contract from Tappe
Associates to do the roof project at the Lynch Park Barn. This has not been
reviewed as yet but is on going.
Chairman Bouchard states that the Beverly High School Field House Roof Project
is now eighty-nine (89%) percent complete by payment and they have reached
substantial completion on this project.
Chairman Bouchard states that the Wind Turbine at the Centerville School is still
being worked on and changing the blades may improve the results.
Chairman Bouchard states that notice has been received from the surety that
directs the City not to make any more payments to AMG until further notice. A
copy of this letter was taken to the Mayor and City Solicitor.
Chairman Bouchard states that a post construction inspection, a one-year
warranty walk through, was done although substantial completion on the
Centerville school was just signed, on a school that has been occupied for at
least a year. Chairman Bouchard read a letter stating several items that need
attention including the wind turbine. Chairman Bouchard will get more
information and go from there.
Mr. Maxner asked if the cladding had been repaired and Chairman Bouchard
states that they came back and redid the cladding, although the letter states that
it is rough and looks unprofessional. Substantial completion was signed off as of

September 24, 2004, and a change order (#16), brings the total for the
contractor to Nine Million Five Hundred Twenty-Seven Thousand Two Hundred
Fifty-Eight ($9,527,258.00) dollars and thinks that this change order is to clean
up the entire project.
Chairman Bouchard presents the following invoices for payment:
1. Tappe Associates 40620 $ 163.36
2. Tappe Associates 40920 174.98
3. SMMA 20527 6,033.08
4. Helfrich Brothers Permit #4 15,025.00
Mr. Maxner made a motion to pay the invoices as presented. Mr. Skokski
seconded. All in favor. Motion passes.
Mr. Maxner made a motion to adjourn the meeting at 7:15 P.M. The motion was
seconded by Mr. Skokski and carried unanimously.
