Attached for your information is a copy of the Technical Proposal for the wetland
delineation for the Beverly Farms Cemetery. As you will note, the bottom line is that the
delineation will cost $675.00. This is not a survey per se, this is only for boundary delineation,
a site report, and a sketch plan for the site.
If you chose to eventually go forward with this project, you will need to get two more
bids due to the competitive bid process. I have other names for you.

Bevedy Farms Cemetery, Haskell Street and Hart Street, Bevedy, MA
Dear Ms. Hudburt:
This letter serves as a Proposal for services to be provided by Hancock Environmental
Consultants, (hereinafter "Hancock") to Beverly Conservation Commission (hereinafter,
"Client"), in connection with Bevedy Farms Cemetery, Haskell Street and Hart Street,
Bevedy, MA flax Map 49, Lot 52). Client endorsement of this Proposal will constitute
acceptance of this Proposal which, once accepted, will become the Contract between the
parties.
OBJECTIVE
Delineate wetlands and prepare site sketch and Wetland Scientist's report.
SCOPE OF SERVICES
Hancock proposes to achieve the OBJECTIVE as follows:
Coordination with Client. Field delineate limits of vegetated wetlands on the subject
property.
Prepare sketch plan of wetland flags to facilitate survey location
Provide site report regarding wetland resources.

Our services will NOT INCLUDE any testing for oil or hazardous materials on subject
property, nor any evaluation of subject property under Massachusetts General Laws, c.
21E. Hancock will be under no obligation to test for, analyze, evaluate or disclose the
presence on subject property of oil or hazardous materials.
ADDITIONAL ASSISTANCE
Aside from the project SCOPE OF SERVICES, Hancock is capable of providing additional
technical services, such as, but not necessarily limited to, topographic survey to locate
wetlands, floodplain engineering analysis or approval process services such as attending
meetings with municipal boards or concemed parties. These efforts are not included
within the SCOPE OF SEP, VICES and would be billed separately as set forth below, under
COST.
SCHEDULING
Hancock is presently in a position to begin work on this project immediately upon our
receipt of the executed contract and a Purchase Order Authorization as set forth below
under PAYMENT.
We estimate completion of the SCOPE OF SERVICES within one (1) week of receipt of
the executed contract.
COST
Professional fees shown under the SCOPE OF SERVICES represent an ESTIMATE
ONLY, not a "quote" and not a lump sum fee. A "quote" may be furnished upon request.
This estimate does not include ADDITIONAL ASSISTANCE as described above, materials
reimbursement, payment of filing fees, application fees, excavating equipment rental and
the like. The actual bill may be adjusted to reflect the novelty and/or the difficulty of the
work, inclement weather/snow conditions or time limitations imposed by Client subsequent
to contract acceptance. Cost estimates for ADDITIONAL ASSISTANCE will be provided
upon request. ADDITIONAL ASSISTANCE will be billed at our standard FEE SCHEDULE,
a copy of which is attached.

The cost estimate and the standard FEE SCHEDULE, as set forth on the attachment, shall
remain effective for one hundred eighty (180) calendar days following the date of this
Proposal.
PAYMENT
Hancock will require a Purchase Order Authorization.
Progress billing is customarily sent on a monthly basis. Payment is due upon receipt of
the bill. Accounts which are not paid within thirty (30) days of the due date will be charged
1% per month. In the event an account is not paid when due, Hancock may, after notice
to Client, terminate all services under this Proposal and may retain the title to and use of
all Hancock work proddcts, including, but not limited to, engineering work, reports,
sketches, plans, surveys and notes, until such time as the account is paid in full. Any
costs of collection of such overdue account, including reasonable attorney's fees, shall be
paid by Client.
USE OF INFORMATION
Hancock shall have the right to utilize all work products, as defined above, obtained during
the course of this Project for purposes unrelated to this Project; provided, however, that
no confidential information shall be disclosed to any person without Client's consent.
LIMITATION OF LIABILITY
Client agrees to limit Hancock's liability to Client and to all construction contractors and
subcontractors on the project, if applicable, to Hancock's negligent acts, errors or
omissions, such that the aggregate liability to all those that may be named shall not
exceed Hancock's fee.
PROJECT ADMINISTRATION
Hancock requests that Client or a specific individual designated by Client be the Principal
Contact on this project. Hancock will report to the Principal Contact, and to the extent
necessary, provide copies of correspondence and seek direction.

Within Hancock, Fwill serve as the Project Manager, and will be the pdmary contact person
for day-to-day activities on the project.
ACCEPTANCE
If this Proposal meets with your approval, please sign and date where provided below,
retain a copy for your records and return the original to me, together with your initial
payment on account. Also, please identify the Principal Contact, (with telephone number)
with whom we should communicate.
This Proposal may be withdrawn by Hancock, if not accepted, as set forth above, within
thirty (30) calendar days of the date hereon.

this project.

Dear Mr. Papamechail,
In response to our telephone conversation, I am writing to you in request of
buying new "NO DOGS" signs, if possible, for our city's cemeteries. The
following would be my recommendations to inform people and to help me
enforce the law. Please let me know if this is something the Public Works
Department can help me with.
CENTRALCEMETERY
* Butman Street side- Has no sign at all- would request a sign on fence (not
on gate, can not see when open) or on pole/post.
* Lothrop Street side- Has a sign on gate but faded-replace??
* Hale Street side- On "Cemetery Rules" sign but some one crossed out the
"NO". Would request painting "no" in again and putting up a separate sign
on the fence!
BEVERLY FARMS
* Haskell Street side- Has sign, all set.
* Haskell Street side (across from High Street)- Would request a sign,
pole/post akeady there.
* Hart Street side- Has sign, all set.
ST. MARYS
Main Entrance- Needs sign, would need a pole/post.
* Second Entrance- Needs sign, can go on the fence.
NORTH BEVERLY '
* First Entrance- Needs sign on tree.
* Middle Entrance- Has a sign, all set
* Last Entrance(closest to building)- Has a sign, all set
The signs already in place are approx. 11" x 17" with a white background, a
red border, and thick, red, print stating "NO DOGS ALLOWED". We
would need a total of seven new signs.
